1. Pre-Election Period (Conceptualization and Formation Phase)
The Christian Health Practitioners Kebbi (CHPAK) was established in response to the recognized need for an organized platform that integrates faith-based values with professional healthcare practice. For the purpose of fellowship, prayer, support, and unity amongst other reasons. The concept emerged in 2019 through consultations among a group of committed health professionals who shared a common vision of promoting holistic healthcare that addresses physical, mental, and spiritual well-being.

It was then known as Christian Health Workers Association of Nigeria (CHWAN) this cause was championed by Rev. Joshua Ochimana and Others. (see the then meeting attendance). It is worthy of note that, prior to this time individuals have attempted establishing such fellowship but, it was not actualized.
During its formative stage, the Association functioned as an informal network comprising Medical Doctors, Nurses, Pharmacists, Medical Laboratory Scientists and other professionals in the Federal Medical Center Birnin Kebbi then, now Federal Teaching Hospital. Activities undertaken in this period were largely voluntary and included periodic meetings.
Key milestones during this phase include:
• Initial stakeholder engagements held in office of the then Deputy Head of Clinical services held on 28th June 2019,one of the classes in School of Optical Dispensing Technology of the then FMC B/K and Christian Evangelical Fellowship of Nigeria, Badariya Birnin Kebbi.
• Formation of steering committee
• Drafting of the Association’s constitution and operational guidelines by constitution committee chaired by Dr. Idami K.
• Preliminary membership drive and database.
These early initiatives underscored the necessity for a formal governance structure to ensure sustainability, accountability, and strategic growth, thereby setting the stage for the transition into a formally constituted body.
2. Election period (institutionalization and leadership transition phase)
The election period marked a critical turning point in the institutional development of the Association. Following the election organized in 2021 and conducted by Dr Hope Abitare assisted by Barrister Elijah Salihu Malgwi ,the following persons were elected as steering committee to pilot the affairs of the association.

General Meeting was convened in 30th Oct. 2021 At CEFN Church. During which the president welcome all on board and encouraged members to be good ambassadors of the association.
This committee started with zero account and functioned for one year beginning from Nov. 1st 2021 to Oct. 31st 2022.
Subsequently, the EXCO organized meetings of stakeholders and general members to actualize its mandate.
Key activities and milestones during this phase include:
• Establishment and sustenance of EXCO, stake-holders and general meetings.
• Visitation of members in moment of Joy and otherwise.
• Introduction of packages for financial members.
• Encouragement of members on voluntary financial contribution.
The emergence of the elected Executive Committee, led by Dr. Tuko Tari Moses (President) marked the formal transition from an informal fellowship to a structured and governed professional association.
3. Post-election period (consolidation and program development phase)
Following the successful one year tenure of the steering EXCO, and considering the achievement recorded within the period, the Association in its general meeting held in Dec. 2022 At CEFN Church endorsed the continuity of the EXCO members for a period of three (3) Years as constitution of Association spelt out for tenure from a motion moved by Chief Benjamin Kawai and seconded Pharm. Adeoti Funsho and accepted by the general house. This marked a phase of consolidation, institutional strengthening, and programmatic expansion under the leadership of the elected executives.
During this period, deliberate efforts were made to formalize operations, expand membership, and enhance the Association’s visibility and impact. The leadership prioritized the development of strategic frameworks and the establishment of functional committees to drive its mandate.
Notable achievements during this phase include:
• Formal registration of the Association with Corporate Affairs Commission (CAC) on 20th Jun. 2022
• Formation of its Board of trustee ( as contained in the CAC certificate)
• Establishment of key committees (e.g., Welfare, Seminar, Blood donation drive, and Outreach)
• Expansion of membership from less than 50 to more than 300 members within 2019 to 2025.
• Development of strategic plan and flow chart of activities annually.
• Implementation of two (2) blood donation drive, three (3) medical outreaches independently and in partnership with Christian Association of Nigeria (CAN) respectively to the communities.
• Establishment of Cooperative society.

The Association also strengthened collaborations with Christian Association of Nigeria (CAN) Kebbi state chapter, faith-based institutions and government agencies thereby enhancing its capacity to deliver impactful health interventions, particularly among underserved and vulnerable population.

4. Current status (operational maturity and strategic positioning)
As of 2025 at the general meeting at the expiration of the first tenure on Dec 2025, the EXCO members were requested to carry on for the second tenure for effective and actualization of the set goal. the Association has evolved into a structured and dynamic organization with a clearly defined governance framework and an expanding portfolio of activities.
The Association is currently characterized by:
• A fully functional Executive Committee led by Dr. Tuko Tari Moses
• Active membership strength of 400 members across Kebbi State.
• Regular implementation of outreach programs, professional development initiatives, and faith-health integration activities.
• Strengthened institutional partnerships with faith based organization and health institutions.
• Increasing recognition within both the healthcare and faith-based sectors
In addition, the Association has recorded the following recent achievements:
• Consolidation on the already established achievements
• Financial support through its cooperative arm

5. Future outlook
Building on its current momentum, the Association is strategically positioned to expand its scope and deepen its impact. Its future priorities include:
• Scaling up community-based health interventions across.
• Development of website.
• Enhancing capacity-building initiatives for members through training and mentorship programs.
• Advocating for policies that support the role of faith-based organizations in healthcare delivery.
• Establishing sustainable funding mechanisms and institutional partnerships.
Conclusion
The Christian Health Practitioners Association Kebbi (CHPAK) has demonstrated a progressive evolution from a conceptual initiative to a formally structured and impact-driven association. Its commitment to integrating faith-based principles with professional healthcare practice continues to position it as a vital contributor to holistic health promotion and community development.
Indeed, thus far God has through individuals made the association what it is today.
The dynamic leadership style of Dr Tuko Tari Moses and his numerous achievements will ever be remembered and highly appreciated.
